The Colostomy Story

Today, I had to deal the dilemma of me being an experienced neurosurgery nurse, working in a general surgery unit. I worked in an ER and neurosurgery environment for the last seven years, and I knew that I would be faced to deal with some unfamiliar cases in a new work place.

 

I had a patient with a colostomy. Well, it may sound so simple for some general surgery nurses out there, but it was a big deal for me. It’s not that it was my first time, because I have had several patients with a colostomy. But it was my first time in several years to actually perform colostomy care – to actually touch and clean the stoma.

 

I was busy with another patient when I heard somebody screaming. When I looked out of my patient’s room, I saw one of my co-nurses coming out from one of my patient’s room. She asked me if the patient in room x is my patient, so I told him yes and asked her what’s wrong. She said the patient was complaining because he was yelling for an hour already and nobody is coming for him.

 

Since I was finished with my current patient, I decided to check the complaining patient out. I went to his room and I was welcomed with some more yelling. I asked him if he was pressing the bell since that is the proper way of calling a nurse. He said he did but it seemed the bell was not working.

 

I investigated further, and found out that the bell cord was not attached to the plug. So that was the reason nobody was coming to him. I was successful in trying to pacify him. I asked him what he needed and found out that his colostomy leaked up to his back. He said he was trying to open his colostomy because he felt it was full of air. When he opened it, he got surprised with the contents and everything leaked out and went to his clothes and sheets.

 

My patient had a colorectal cancer and the surgeons tried to remove the cancerous part of his large intestines. A new opening was made on the left side of his abdomen where his stool can come out, and will be drained to a pouch called colostomy bag.

 

I checked what I needed to clean him, and after that I told him I need to get some things in the stockroom. When I got into the stockroom, I took some fresh linens, pads, cleaning wipes and saline. To my horror, I found that there were different sizes of colostomy bag. How would I know which one is for my patient? I decided to just bring one from each of the different sizes. I thought it would be safe since I do not have to come back if one size fails.

 

When I went back to my patient’s room, I checked his colostomy so I could choose which size of bag I would need. To my surprise, none among those I brought were the same as my patient’s. I needed the biggest size which seemed not available because I did not find that size before. I knew I took one sample from each of the sizes I saw.

 

I went back to the stockroom and searched for the largest size. I did not find any, so I asked one of my seniors. He helped me search until he finally said it was probably out of stock.

 

I decided to go to another ward and asked for that size. I knew that every minute counts for my patient who was irritable initially. I found the size from another surgical unit and hurriedly went back to my patient.

 

When I got into my patient’s room, I was greeted by a frown. I explained to him why it took me a bit long to come back, that I needed to get his size from another unit. He seemed dissatisfied with my explanations, so I just tried my best to talk to him nicely.

 

I detached the old pouch from where it was connected. I placed it in the bathroom since I wanted to save the wire that closes the pouch at the end. I cleansed his stoma with normal saline. After cleaning, I tried to attach the new pouch to the connector. It seemed I could not connect it. It was either I was doing it the wrong way, or I just do not know what I was doing.

 

While I was trying to attach it, I tried to converse to him to try divert his attention from what I was doing. I explained to him some facts about colostomy, and how to take care of it. I was posing to be a pro with what I was doing, when deep inside, I knew I could not connect the bag. It was hurting him when I pressed harder on the pouch lid.

 

I then decided to ask help from a co-nurse. God probably heard my heart beating fast, and gave me a colleague just passing by my patient’s door! I asked her to help me attach the colostomy bag. She told me how to do it and watched me do it. It was the same technique as what I was doing before. But she also felt it was difficult doing it that way so she suggested for me to just remove the part that was attached to the skin, and apply a new one. The adhesive part that was touching the skin and the bag should already be connected before sticking it to the skin.

 

I actually thought of that, but I knew it was the harder way of changing the bag. But now, it seemed there was no other way but to do it that way.

 

I was successful in applying a new bag on him. I went back to the bathroom and took the wire from the old bag. It was heavily stained with stool so I just tried to clean it. I closed his colostomy bag using that wire.

 

I wiped him a bit, and brought him to the bathroom after that. I changed his gown and placed new linens on his bed. His mood has changed, and was apologetic about his attitude a while ago. I told him it was pretty understandable for him to get mad in his situation. I left him clean and satisfied with what I have done for him.

 

In the evening, before I left the unit, I checked him out in his room. He was with his family, and I was introduced to them. I told him that my shift is over and that I was leaving. He asked who’s replacing me, but I was not sure who’s taking care of him next. I saw in his face that he still wanted me to stay for him.

 

Before I left, he said smilingly “See you on Sunday!”.

    I could not imagine myself getting into a surgical unit because i really hate the messy stuff such as wounds, colostomies, bedpans. Reading this, i decide that i will not leave ER 🙂

    Yaiiiiks! Ang yucky naman ng topic mo ngayon! But this only shows how difficult our work is as nurses. Kasi akala nung mga nag aaral pa lang, pasosyalan lang ang nursing. Dami kayang kadiri sa work naten… Hirap kumita ng pera noh! Hehehe! Stay cool and humble!

    On behalf of ostomy patients everywhere, thank you for going the distance for your patient I am an ostomate of 17 years and the president-elect of the United Ostomy Associations of America. We get calls and emails frequently from patients who’ve had bad experiences with health care professionals who are not specialized in ostomy care and are ignorant to the needs of the patient with bowel or bladder diversions. Your ability to remain calm, despite your own apprehensions, surely did wonders to increase this gentleman’s comfort and acceptance of his ostomy, and make what could have been a terribly embarrassing and uncomfortable interaction a positive one.

    I am a little clueless about colostomy bags and was asked by a cousin to buy one. I was surfing to find out whether it does come in sizes and your article answered my question: that they indeed come in sizes. Now, I just need to find out the exact size of my uncle’s pouch.

    I want to commend you for going the extra mile to help the patients. Kasi, before I went online, I called a Mercury Drug branch trying to find out for the STANDARD colostomy bag sizes. The lady who answered kept saying this is our AVAILABLE sizes. I then, told her that regardless of what they have on-hand, I wanted to know the STANDARD sizes. I said goodbye and put down the phone thinking: either she doesn’t know the difference between STANDARD and AVAILABLE sizes, or she’s one of those many who is not very much into helping people.

    Not everyone can handle such pressure. A lot of patients become irritable in their discomforts, and it is understandable that a nurse must keep his composure. I’m glad you’re a nurse who doesn’t make snide remarks or answer the patients rudely when in that situation.

    Magandang araw po sa inyong lahat! Nais ko pong ibahagi sa inyo itong aming naging karanasan para sa aming anak.
    Ako po si Elbert Catacutan, Ama ni Jehdeiah Elbert G. Catacutan; siya ay ipinanganak sa Baptist Aklan Hospital noong June 18, 2011. Gusto ko pong ishare ang aming naging karanasan nang siya ay ipanganak na may birth defect gaya ng kawalan ng butas sa puwet: tinatawag ng mga doctor na ‘imperforate anus‘.

    -Masayang masaya ako nang ipanganak si baby Jed. Ngunit ang kasiyahang ito ay napalitan ng kalungkutan, dahil sa ospital ay hindi napansin ng mga nurse o ng doctor na nagpaanak sa asawa ko na walang butas ang puwetan ng oras na iniluluwal sa sinapupunan ng asawa ko si baby Jed, nandun ako sa loob ng operating room para magbigay suporta sa kaniya. 12 hours ang lumipas hindi pa dumumi si baby hanggang umabot ang dalawang araw hindi pa din nakadumi hindi na nag usisa ang mga nurse kung bakit wala pang dumi kaya dinischarge na kami at pinauwi sa bahay. Nangamba ako pagdating sa bahay kasi alam ko na lason yun kaya dapat ilabas yun ng baby.. Ala una ng madaling araw halos hindi kami makatulog ng asawa ko, tinignan ulit namin ang diaper nya kung may dumi nagsuspetsa na ako na baka walang butas ang puwet kaya tinignan ko at wala nga gaya ng hinala ko dali dali kami nag impake at tinawag yung kaibigan ng bayaw ko na may sasakyan para arkilahin at magpahaqtid sa ospital ngunit umabot pa ang isang oras wala pa din makontak kya pinuntahan ng bayaw ko yung isa pa niyang kilala pero lasing daw kaya hindi kaya magmaneho pero nung mabalitaan niya na kailangan namin dalhin agad ang bata sa ospital ay sapalaran siya tutal daw medyo wala na yung hilo niya. sinundo nya kami sa bahay at hinihatid sa ospital ng ligtas mula sa malayong lugar na mabundok . Dali dali naman ipinatawag ng nurse si doc. nang malaman nila na walang butas ang baby at ng makita ni doc nagulat siya at medyo kinabahan kaya sinabihan niya kami na irerefer sa Kalibo. Sa oras din yun naglagay si doctora ng NGT sa baby para hindi magsuka dahil medyo matigas na ang tiyan at malayo pa ang biyahe. Malakas ang ulan ng mga pagkakataong iyon pero naghanap ako ng Van na masasakyan namin papunatng Kalibo Provincial Hospital.

    Dumating kami sa Provincial eksaktong 1las 6:00 ng umaga at nag pa confined na agad kami dala ang refferal letter. Nag-aalala kami dahil ilang oras pa ang lumipas ay hindi pa din dumating ang doctor surgery. Ilang bese ako nagtanong tanong sa mga nurse at sa wakas alas onse ng umaga ay dumating si Doc.Tinignan niya yung kundisyon ng bata. kung may konting butas ba yung puwet o talagang wala. habang dinadiagnose ni doc ay lubhang nag-aalaal ang asawa ko at nag aalangan kung itutuloy pa namin ito dahil halos walang pahinga at matamlay na hinang hina na siya mula ng ipanganak na ang bata at wala halos tulog at pahinga.
    Hindi ko ipinakita sa kaniya na ako man ay nanghihina din ang kalooban. Sinabi ko na kailangan ituloy ito kung hindi mamamtay ang bata at konsensya natin ito kung hindi natin agad ipapaopera.

    Ang totoo kasi, hirap din kami sa financial kaya naghalo na din ang takot at pangamba kung saan kami hahagilap ng pera. Kahit na di alam kung saan kukuha ng pera ay ipinatuloy namin ang operasyon Sabi ko sa asawa ko hindi tayo pababayaan ng Diyos, hapon tinignan kung may konting butas kaya pina x-ray ang baby, posterior at lateral position pati na din inverto gram takot na takot at awang awa sa anak ko dahil sa mura niyang edad na halos wala pang isang linggo ay ganun na ang dinanas.nanginginig ang kamay ko habang hinahawakan siya ng patiwarik para sa invertogram.. hindi lang tatlong beses ginawa dahil sobrang lakas ng bata at pumipiglas totoong tumutulo ang luha ko, sa pagkakataong iyon hindi ko na pinasama asawa ko nanatili na lang siya sa room para hindi mag alala. Pagkatapos ng resulta ng x-ray agad akong kinausap ni doc. ukol sa surgery at ipinaliwanag niya sa akin ang gagawin niya. dahil walang butas sinabi niya na may tatlong type ng ganito. 1: low type, 2: medium at panghuli ang high type. kung low type at may butas mas madali daw ito pero dahil walang butas kahit konti nakita sa result na high type. Kaya kailangan gumawa ng colostomy. Nagkaroon kami ng konting pag asa dahil sabi ni doc hindi naman daw ganun kahirap gawin iyon at less fatal at well experienced na siya sa mga ganung kaso inalakala namin na kami lng ang may ganung kaso pero sabi niya napakadami din ganyan hindi lng sa bata pati din sa matanda.Natulala ako nang sabihin niya yunat kahit paano ay binigyan kami ng lakas ng loob.

    Kinagabihan ng alas onse ay dinala na siya sa operating room. Naghintay ako sa labas hanggang matapos ito halos dalawang oras At sa wakas ay lumabas si doc. at sinabi na ok na siya tapos na ang operasyon. Yung sobrang kaba ko ay napalitan iyon ng konti kasiyahan bagaman ang totoo maraming pa siyang hirap na mararanasan. pagkalipas ng kalahating oras ay nilabas na si baby Jed sa operating room at dinala sa kwarto.Pagkakita namin kay baby muli na naman kami nanghina dahil sa dami ng tube na nakalagay sa kaniya. Mayron siyang NGT at Oxygen,Tapos may dextrose tsaka tither sa kaniyang ari bukod pa sa kaniyang stoma sa tiyan. Tulog si baby sobra isang araw na hindi minumulat ang mga mata at naninilaw kaya nilagay sa incubator na may walong ilawan para mainitan ang balat. Nangamba agad ang asawa ko at iyak ng iyak bakit daw ito nangyari sa amin lalo na sa aming anak. Sabi ko may layunin siya kung bakit niya ito itinulot sa atin lakasa mo lang ang loob at laging magtiwala sa kaniyang banal na magagawa. Umabot kami nang halos 2 linggo at araw-araw ay madaming gamot na nilalagay sa kaniya. Lumobo ang bill namin sa ospital maliban pa sa private doc na nagopera at mga kasama niya. Sa awa at tulong ng Diyos ay bumuhos ang madaming tulong mula sa aming mga kasamahn sa trabaho bagaman maliit ay malaking tulong na din at nakabawas ng alalahanin at maging ang aming mga mahal sa buhay ay tumulong din financial man o hindi… Salamat na lang at may Philhelath kaya medyo nakabawas din ng bayarin..

    Sa ngayon si baby ay nagpapalakas na dahil sabi ni doc ay muli siyang ooperahan para ilipat sa pwet pag nasa hustong gulang na siguro pag nasa 2years old na. Tinanong ko agad si doc kung magkano kaya ang magiging gastos para dun. Ang sabi niya maghanda na lang daw dahil medyo malaki yun at marami naman na institusyun na maaring lapitan para sa mga taong may ostomates kagaya ni baby Jehdeiah.. Upang makatipid kami sa pag irrigate ng kaniyang dumi ay tinuruan kami ni doc. na gumawa ng colostomy bag. ginawa ko yung bigkis ng bata nilagyan ko ng butas sa gitna na sakto sa kaniyang stoma, nilagyan ko ito ng plastic ng tubig at nilagyan ng hypoallergenic micropore tape para hindi mag infection at ointment na calmoseptine cream para sa paligid ng kaniyang stoma. Sa isang araw ay halos 6 hanggang sampung bigkis na improvised colostomy bag ang nagagamit ni baby. Totoong matrabaho at kailangan ng sakripisyo. dahil pareho kami nagtatrabaho bagaman si misis ay naka leave pa din.Kailangan namin mag ipon para sa susunod na operasyon ng aking asawa. Bago kami bumalik sa trabaho ay itinuro namin mabuti sa aming biyenan na sanay na sanay mag alalga ng bata dahil dati na siyang namasukan bilang kasambahay.

    Salamat sa Diyos na patuloy na gumagabay sa amin sa kabila ng mabigat na suliranin na aming naranasan; sabi nga ” Hindi tayo bibigyan ng pagsubok nang ating Panginoon Diyos kung hindi natin ito kaya” Magtiwala at umasa parati sa kaniya..
    Nawa’y nakapag dulot po ako ng konting inspirasyon sa sinumang makababasa nito na may ganito din karanasan.maari ninyo din pong ishare at magtutulong tulong po tayo ukol dit sa abot ng ating makakaya. Anu man pong comment at suggestion ay malugod ko pong tatanggapin lalo na po at may susunod pa na operasyon para kay baby na aming pinaghahandaan.

Napakasuwerte ni Baby Jed dahil mayroon syang magulang na hindi basta sumusuko. Alam kong marami pa kayong pagdaraanan ng iyong anak at asawa. Pagkatapos maisagawa ang operasyon sa kanyang puwitan, ang susunod naman na operasyon ay ang pagsasara ng colostomy. Dalangin ko na bigyan pa kayo ng ibayong lakas at tatag upang maharap ang mga pagsubok sa inyong buhay.

        Medyo matagal ako hindi nakapg post dito anyway as of this time waiting na kami ni baby Jehd sa final operation this coming Mrch 2013. Sobrang likot na ngayon ni Jehd at talagang takbo ng takbo sa Beach. madalas yung damit niya mapuno ng popo dahil malikot kumilos pero ok lang sa amin.

            Sir elbert the same story po tayo ganyan din sa anak ko untill now d pa na operahan sa pangalawa at pangatlong opera mag 2years old na si baby den this coming nov 11 2016.. ang nadaanan mong pag alala at kahirap pariho tayo..nong pag xray na nang anak ko doun din ako parang gustong bumigày pero nilakasan ko loub ko kasi ang asawa baka d rin nya kakayanin. Sa awa nang dios nandoun c doc ybañes isang napaka bait na doctor kasama nang kanyang manga kasamang nurse dahil lagpas na sa oras kasi 2dàys na ang bata d tumae d na nya kami ni refer sa cagayan de oro general hospital sapagkat ozamis city pa kami manggaling..d ko alam kong pdea specialist s doc kasi sabi nya malalaking na daw na operahan nya nang ganon. Pero thank full kami kasi pinapriority nya kami sa pag bakante nang OR ksi siningitan nya ang naka schedule sa OR tas d pa nya duty that time tinawagan lng cya sa clinic nya…maraming salamat sa manga doctor at nurses isa kayo sa manga buhay na hero….

    ako si weng rubi ng san mateo rizal, gaya ng kay elbert nanganak ako ng march 21 2012. si jhen rowen rubi ang name nya pinanganak ko din sya na walang butas ang pwet katulad ng reaksyon ni elbert natakot at nagtanong di ako kung bakit ko sya naipanganak na my deperensya samantalang lahat ng aking anak ay normal kong pinanganak. tama my plano si lord sa lahat ng bagay na binibigay nya sa atin at hindi nya ibibigay sa atin kung hindi natin kaya ang mga bagay na ito. sa ngayon sa tagiliran sya dumudumi nahihirapan din kami sa pagbili ng colostomy bag dahil masyado mahal at di namin kaya bumili palagi ng colostomy bag. hindi man lang kmi tinuruan ng doktor kung pano gumawa ng artipisyal na colostomy bag.       Hi ROWENA. Natutuwa ako dahil sa kabila ng pagsubok na dumarating sa iyong buhay, nananatili kang matatag at naniniwala na ang lahat ay nakaplano ayon sa kagustuhan ni Lord.         isang mapagpalang araw po ,pasensya na po at ako ay ngayon lang nakapagreply . ngayon kolang nabasa na kayo po pala ay nagreply sa aking sulat. hanggang sa ngayon po ay nanatili po kaming matatag mag-asawa at mga anak ko sa binigay sa amin ni lord na si jhen rowen. dipo sya pedeng operahan at mabutasan ang kanyang pwet hangang di pa naaayos ang kanyang puso nakita po kasi sa kanyang medical result na ung apat na butas nya sa puso ng sya ay ipinanganak lumaki ang isang butas. sa ngayon po ay nakapila kami sa heart center.para sa kanyang puso. pero mabait parin sa amin si lord. 2 years old na si jhen ngayon kung titingnan mo sya ay para syang walang sakit.kaya lang naawa na kami dahil alam ko hirap narin sya sa colostomy nya minsan kasi my irritaion dahil sa sugat sa paligid ng colo nya. maraming salamat at mayroon ganito website na nakakaibsan ng bigat ng aming nararamdaman at mayroon kaming napapagsabihan ng aming problem sa buhay salamat po…………… god bless us

            more than 20 yrs before ako naging maayos dahil hinintay pa ng pamilya ko bago ako maoperahan pero sa tingin ko mas maganda na ngayon ang mga gamit ng hospital kumpara nun mga early 90’s at isa pa mas magagaling na ang mga doctor ngayon kesa nun

      Thanks for the reply.
      Kung need mo pa ng help kung paano gumawa ng colostomy bag. yung bigkis ng bata lagyan mo ng butas sa gitna tapos ifold mo yung plastic ng yelo at lagyan ng tape para di magleak. yung cream na ginagamit namin ay calmoseptine. 35 pesos kada isa pero konti konti lng pag gamit nmn nun. mabisa yung cream na yun. kapg madalas nmn na basa yung popo nya lagyan mo lng ng bulak sa loob ng plastic pra ma absorbed niya then palitan agad kapg may dumi na para hindi mag irritate yung skin.

      Good news for my son Jehdeiah Elbert although his second operation for PSARP was revised due to leakage sa iloilo doctors hospital noong nakaraan April nilagyan siya ng butas sa pwet tapos inulit nga kya nagdoble gastos dahil dumeretcho yung popo nya na hindi pa dapat daanan. isa pa madalas mag prolapsed yung coloctomy niya. kaya nung June dun din sa hospital na iyon ay inayos ni Doc ROBERTO LOZADA. Pediatric Surgeon. Kada buwan pumupunta kami sa clinic para palitan yung dilator niya na stainless para hindi mag close yung anus niya. Araw araw ginagawa ko yung pagdidilate. Sa ngayon naghihintay kami sa final operation niya dapat sana ay sa NOV 2012 kaya lang pinamove ni DOC ng March 2013 para mas mahaba daw ang preparation at mas maganda ang paghilom ng pwetan niya. Sa ngayon siya ay lumalakad na at parang walang deperensya.
      I thanks God for being good to us even though the trials are so difficult; but i know he always guided us.
      Let us put our trust in him no matter what happened in our life co’z God has always in good purpose.

      mas malaki po ang pwede ko ma advise sa inyo dahil 20 yrs ko po dinala ang kapansanan yan na nagbigay sakin at nagdulot ng pagkahiya sa lipunan andun yun hindi mawala panlalait ng ibang bata at tuksuhan sa bawat taong nakakalam ng kapansanan mo .kaya malaking dagok sa anak ninyo ang problemang yan kaya pls hayaan nyo mabigyan ko ng kahit ano klasing turo oh gagawin kayo sa anak nyo

    Hi Nelson,
    When you began to follow my blog, I came over here to check out yours and found this post.
    I had surgery for what was thought to be colitis (it’s actually Crohn’s) in 1982 — while they joined up my ileum to my rectum, I had a colostomy and a bag for a few months.

    Let me tell you, you handled the situation beautifully. Seriously. As a patient, it is scary to be dealing with something so different when you don’t know what to do and you don’t really want it to begin with! Calm, efficiency always helps. And you know, if you don’t know what to do, asking for help rather than making the patient suffer longer is exactly the right thing to do. As I recall, the stoma site burns and itches terribly.

    may anak din akong may imperforate anus feb 23 2016 ko siya isinilang,tanong kulang sabi kasi ng doctor na kadalasan daw ng may ganitong case ay naging mongoloid ang bata…tanong kulang sa inyo na may case na ganito..totoo ba?kasi wala pa kasi 1month ang anak ko..at natatakot ako sa sinabi ng doktor..kawawa naman anak kung ganun.
